Our Facilities
The CSSS des Collines provide services in five different facilities: the Wakefield Memorial Hospital, the Centre d'hébergement La Pêche, the CLSC and its 4 points of service. Since 2006, an innovative residential project in the community can be added to the list. This residence is located in the Manoir de Wakefield.

The CLSC des Collines:
The CLSC provides services from its four points of service located in the four municipalities of the territory, to a population that is for the most part at home, in school or in daycare centres. In addition to home support for adults suffering from loss of autonomy, the CLSC provides perinatal and early childhood services, services to young people in difficulty, mental health services to young people and adults, services for persons living with an intellectual disability or a pervasive developmental disorder, front-line health services and community organization services:

Diversified residential services in the community:
The population of the territory can also count on three family-type resources totalling 19 places for elderly persons with autonomy loss and two intermediate resources totalling 10 places, which accept persons requiring a more intense level of services.
Two resources are located in Cantley, one in Duclos, one in Val-des-Monts and one in Wakefield. The Manoir de Wakefield is a private residential residence which, through a service contract, reserves 10 residential places for our clientele suffering heavy autonomy loss. This is an interesting example of partnership between the public and private sectors.
The CSSS des Collines comprises a large team of devoted and competent staff members. 18 physicians, 8 consultants and a pharmacist form the medical and pharmaceutical team. 275 employees, 7 middle managers and 5 senior managers, in addition to the executive director, work assiduously every day either for or in direct contact with the clientele. Assistants, clinical supervisors and unit coordinators complete the team.
